Must be 18 or older to fill out a waiver. Waivers of minors must be filled out by their PARENT or LEGAL GUARDIAN ONLY. NO EXCEPTIONS.

Release Waiver

All jumpers and non-jumpers on the premises of Altitude Trampoline Park must sign a waiver. Jumpers under the age of 18 must get parent/guardian consent and sign-off.
